We make every effort to ensure obtaining your medical records is as easy as possible. To obtain your medical records you can follow these simple steps:
- Open and print the one page PDF form by clicking here. Note: If you need to install Adobe Acrobat reader, please click on the link at the bottom of this page.
- Fill out the form. All fields with an asterisk (*) must be filled out in order for your request to be honored.
- Fax the form to (407) 423-2789.
